Cigna Dental HMO Plan

Eligibility

You must be under age 65, a member of your association for a minimum of 30 days prior to your effective date of coverage and employed in the arts and entertainment industry.

Enrollment Dates

New enrollments for the CIGNA Dental HMO are accepted on a monthly basis. Applications must be recieved prior to the effective date.

This is an HMO PLAN ONLY. You are limited to the dentists in the CIGNA Dental Care Network. Please go to www.cigna.com to find a dentist in your area.

The CIGNA HMO Plan is not available in New Jersey.

If you would like to apply for coverage, print and complete the Information and Rate Sheet and Enrollment Form and return to TEIGIT with the appropriate payment. Applications must be postmarked no later than your effective date.
